We have provided the necessary forms for you to make your first visit with us a little easier. Please click on the links to the right to print off the forms and have them filled out before you come in for your first appointment.  If you are not able to print the forms we can either fax or email them to you.  If you would prefer you can come 20 minutes before your scheduled time to fill out the paperwork.  

* If you are coming in for a Community acupuncture treatment, you do not need to fill out the paperwork to the right, please come in 10 minutes before your scheduled appointment to fill out your paperwork.
